Date Type Regulations Status
2026-04-21 Complaint 13A.16.07.01 Corrected
Findings: Complaint states that a staff person is fussing, cursing, and putting hands on the children. It was reported she pushed a child's head and using the F word. One time she was yelling at the children and turned around and knocked over a bookcase. it was also reported that two teachers were fighting in front of the children. Findings: Interviewed several staff no one stated that they have heard a staff person yelling or cursing. One staff did state that they raise their voices when children aren't listening. Center is reminded that staff must keep their cool and refrain from yelling and cursing. As far as the staff fighting in front of the children, there was an argument between two staff regarding comments on made about another's child. It was during nap time. They are not permitted to fight in front of any children. They need to present a calm demeanor around the children. Appropriate behavior with the children needs to be reviewed with the staff and appropriate alternatives to raising voices need to be given.
2026-03-20 Complaint 13A.16.08.02B Corrected
Findings: The staff person who was working in the classroom where the child was left is an aide. There must always be a teacher qualified staff with each group. All rooms had qualified staff at the time of the inspection. Center needs to submit a new staffing pattern to be sure this aide is not alone with children in the morning before the teacher arrives.
2026-03-20 Complaint 13A.16.08.03D(1) Corrected
Findings: A group of 3 two year olds from classroom 118A was mixed with a group of 5 infants in classroom 118B. The group of infants (all one year or above) had just come in from outside. The teacher in 118A was in the hallway fixing the paper on her door. Her one year old son was with her. That left the teachers with the other 4 infants watching the 3 children from 118A as well. The teacher from 118A needed to stay with her group as it made the infant classroom out of ratio and group size. The infants were in 118B just a couple of minutes but that room is a preschool room and not meant to have children under 2 in it. Infant staff need to remember to be sure that those children are only in rooms approved for children under 2 years of age. This was corrected when the teacher for 118A took over her children and the infants went into their own room.
2026-03-20 Complaint 13A.16.03.03B(1) Corrected
Findings: Complaint states that a child was left unattended in a classroom and are not noticing when children are leaving at the end of the day. Findings: Center did not know child was in the classroom. The child's older brother had dropped him off into an empty classroom. Staff were able to say that children are signed out at pick up time using the classroom clipboards. However the children were outside today during the inspection and the staff did not have their clipboards with them. Specialist spoke with staff and reminded them that the classroom attendance needs to go with them whenever they are not in their classroom so that they can always check it to be sure they have all the children they are supposed to have and aren't missing any.

2025-02-25 Complaint 13A.16.08.01A(1) Corrected
Findings: Complaint states that a special needs child didn't have his socks and shoes and coat on when outside. Findings: Spoke with the staff with the child. The group of 11 children was going outside and one of the teachers needed to use the bathroom. The child was handed over to the 2 year old teacher along with his things as she had space available in her group. It was stated that the other staff person just sent him out the door with the group and she didn't notice that the child didn't have socks/shoes on. At that time his usual teacher asked a teacher that was also outside with her group to keep an eye on her group while she went to get the child's socks and shoes. The parent arrived as the staff person was walking in to get the items. Center is reminded to always be aware of each child's needs and attend to them.
