Noncompliance Area: During the Renewal Inspection on 7/1/2026, the fire and fire detection log was up to date. However, the log showed two lapses in the requirement to conduct a fire drill at least once every 60 days since the previous inspection as follows: Fire drill conducted on 9/8/2025 and not again until 11/10/2025, and a fire drill conducted on 3/9/2026 and not again until 5/11/2026. The log also showed three lapses in the requirement to manually test the fire alarm at least once every 60 days as follows: Alarm was tested on 11/8/2025 and not again until 12/10/2025, alarm was tested on 2/6/2026 and not again until 3/9/2026, and the alarm was tested on 4/8/2026 and not again until 5/11/2026.

Correction Required: Fire drills are conducted at least once every 60 days. Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The fire alarm system must be manually tested and documented at least once every 30 days.

Correction Required: At renewal, the legal entity shall submit clearances for each individual 18 years of age or older who resides in the facility at least 30 days in a calendar year if the individual attained 18 years of age following the date of the previous application for a certificate of compliance. The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). TIERED LIS: 1. Household member #2 may not be present at the facility while children are receiving care. This portion of the plan requires an immediate correction date. 2. All individuals 18 years of age and older who reside in the home at least 30 calendar days per year will have all required clearances on file at the facility in compliance with the regulation and the CPSL. Noncompliance Area: During the Renewal Inspection on 5/7/2025, the Certification Representative reentered the facility at 11:00 AM, the 5 children in care were observed to be unsupervised in the childcare space. Staff #1 returned to the childcare space at 11:02 AM. Staff #1 stated that she went to the kitchen to get food for the children.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

The group consisted of 2 infants, 1 young toddler, 1 older toddler and 1 preschool child. This mixed age group of children requires two staff persons to maintain the required staff to child ratio.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements at §3280.52(b). The required staff:child ratios must be maintained at all times.

Description: Locked or inaccessible - in original container

Noncompliance Area: On 5/7/2025, a container of Febreze was observed to be on the floor next to the toilet in the bathroom that is used by children. The Febreze was labeled keep out of reach of children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.
