Noncompliance Area: During the Renewal Inspection on 4/8/2026, Children #1, #2 and #3 each had current health reports with the immunization history in accordance with the recommendations of the ACIP on file. However, these initial health reports with the immunization history attached were not received within 60 days following the child's first day of attendance at the facility. Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: On 3/19/2026, Child #1 burned their hands on a baseboard heater in the facility while Staff #1 was vacuuming the area rug in the childcare space. Staff #1 is no longer employed at the facility.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times.

Description: Name on bottle/Written consent

Noncompliance Area: On 3/19/2026, Child #1 burned their hands on a baseboard heater in the facility. Staff #1 applied Trombosil cream on the child's hands. During an unannounced inspection on 3/24/2026, it was discovered that the Trombosil cream was not labeled with the child's name and the parent did not provide written consent for administration of any medication.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. A parent shall provide written consent for administration of medication or a special diet.

Noncompliance Area: During an unannounced inspection on 3/24/2026, the record/emergency contact information on file for Child #1 did not contain signed parent consent for emergency medical care for the child and also did not contain signed parental consent for administration of minor first-aid procedures by facility staff. Correction Required: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. Noncompliance Area: On 3/19/2026, Child #1 burned their hands on a baseboard heater in the facility. The parent was not provided with the accident/injury report until 3/23/2026.

Correction Required: A child's record must contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Noncompliance Area: Staff #3 did not complete mandated reporter training within a 60-month time period. This is evidenced by the previously documented mandated reporter training on file being dated 1/28/2021 and the updated mandated reporter training on file being dated 3/11/2026.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). All staff must complete mandated reporter training at least once every 60 months.

Noncompliance Area: During an unannounced inspection on 3/24/2026. Staff #2 was observed to be working unsupervised and directly with children. Staff #2 did not have the completed National Sex Offender Registry (NSOR) clearance on file. See LIS code sheet.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff person #2 may not work in a childcare position with direct contact and routine interaction with children until completed National Sex Offender Registry (NSOR) clearance is on file.

Description: HS/GED + 2 yrs/Primary staff person present

Noncompliance Area: There was an incident at the facility on 3/19/2026 where a child was injured. It was verified that Staff #1 was the only staff person present at the facility when the incident occurred on 3/19/2026. During the file review on 3/24/2026, Staff #1 had a high school diploma on file, but did not have verification of 2 years of experience working with children on file to verify that they were qualified as a primary staff person. Correction Required: A primary staff person shall have attained a high school diploma or a general educational development certificate and 2 years of experience with children. A primary staff person shall be present in a facility when six or fewer children are in care, unless the staff-child ratio specified in §3280.52(b) or (c) requires a second or third staff person.

Description: Mixed age levels

Noncompliance Area: Upon arrival to an unannounced inspection on 3/24/2026, Staff #2 was observed working alone in the Main Room with a mixed-age group of 1 infant, 1 young toddler and 3 preschool children. Two staff persons are required to maintain the required staff-to child ratios for this mixed-age group of children. After a few minutes, Staff #4 returned to the Main Room and had one child with them.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements at §3280.52(b). TIERED LIS: 1. The required staff-to-child ratios must be maintained at all times. This portion of the plan shall have an immediate correction date. 2. The operator must print and post the required staff-to-child ratios in each measured childcare space. The operator shall provide a date for when this will be completed.

Description: Heat Source

Noncompliance Area: On 3/19/2026, Child #1 burned their hands on a baseboard heater in the facility. The child sustained second degree burns on the palms and hands.

Correction Required: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Noncompliance Area: During an unannounced inspection on 3/24/2026, it was observed that the facility was using a small portable space heater in both childcare spaces. The manufacturer's instructions for use for these two small portable space heaters were not in an accessible area in the facility or on file anywhere in the facility on 3/24/2026.

Correction Required: Portable space heaters, if allowed by local ordinance, may be used while children are in care, if the units are used in accordance with the manufacturer's operating instructions. The manufacturer's instructions for use shall be kept in an accessible area in the facility.

Description: Clean, age appropriate

Noncompliance Area: During the Renewal Inspection on 7/2/2025, the playpens and rest mats used by the children in the facility for rest equipment were not labeled were not labeled for the use of a specific child to be used only by the specified child.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.
