However, the log showed that the fire alarm went more than 30 days without being manually tested and documented multiple times during the previous 9 months as follows: Tested on 8/1/2025 then not again until 9/2/2025, then not again until 10/3/2025, tested on 12/2/2025 then not again until 1/5/2026, and tested on 4/1/2026 then not again until 5/2/2026.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The primary staff person or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. The fire alarm system shall be manually tested and documented at least once every 30 days.

Description: Unsafe Areas in Outdoor Space/Building Surfaces

Noncompliance Area: During the opening inspection on 4/11/2025, the following was observed on the back part of a garage located inside the play area: Two broken windows, cracked wood with sharp edges, and nails protruding. In addition, the following was also observed on the ground in the outdoor play area: a steel pipe, a steel shovel and a steel rake.

Correction Required: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions. Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
