The K¹² at the Y program is a supervised, online-learning education alternative, offered onsite at most all Valley of the Sun YMCAs, and administered by accredited Arizona Virtual Academy. Every enrolled student receives a YMCA Youth Membership, free of charge, and all course materials from books to microscopes are provided for use.

Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 3/11/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. The following was discussed, but not limited to: 1) information required on the Child’s emergency cards, 2) Upcoming Licensee Anniversary fee due 3) Ensuring non-enrolled individuals (adult gym members) are not allowed access to licensed sanitary units during camps when children are using them. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.
INSP-0100826 2025-03-13 Compliance (Annual) Complete
Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 3/13/2025 and are subject to changes pending programmatic review. Please submit the current City of Phoenix fire permit when obtained. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was emailed to the Provider at the time of the inspection.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 4/22/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 2 of 2 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Cleanliness of the white refrigerator in Esports. 2. The missing post tops and the sports court in the outdoor activity area. Vehicles CK73539 & CC80320 were inspected and are approved to be used for enrolled children. Compliance Officer is Tricia Tartaglio
