|Findings: At the time of the inspection, the staff to child ratio in the primary bathroom observed by Surveyor #2 was 0:1 (preschool-age child); a staff member was required to maintain the required ratio.|

|Findings: In the 2's classroom, there was a bottle of Windex on the surface of the diaper changing table. In the Infant room, there was a plastic water bottle attached to the faucet of the diaper changing hand washing sink with a pink pipe cleaner.|
|Findings: The Surveyors' observed the following soiled clothing containers lacked a waterproof liner: *the Infant room *Toddler 1 *Toddler 2|
|Findings: In Toddler 3, the soiled diaper container was observed on the floor next to the diaper changing changing station, accessible to the children.|
|Findings: The Surveyors' observed the children in Toddler 3 eating Ritz crackers and drinking water. The enrolled children were not served a second component during snack, as required|
|Findings: Surveyor #2's observed the refrigerator thermometer in Toddler 1 revealed a temperature of 50 degrees.|
|Findings: In the Toddler 1 bathroom, the first toilet had yellowish water and the second toilet had a yellow ring around bowl of the toilet.|

|Findings: The roster in the Infant room noted one child as present, but there were three children in attendance. The roster in Toddler 1 (one and two-year-olds) room noted twelve children were present, but there were eleven children in attendance.|
