Findings: After a review of 3 staff files, it was determined one file was lacking: Staff #3 (start date 7/27/15) had a copy of a fingerprint clearance card application dated 7/30/15, but no copy of a clearance card. Staff #3 was asked to go get their fingerprint card by Staff #1 during the inspection. Staff #3 returned with the rolled fingerprint card. Staff #3 stated " that is what DPS returned to me." Upon further review of the application it was determined Staff #3 was 17 years of age when the application was sent into DPS. Staff #3 did not understand the paperwork that was returned to her by DPS and did not inform the facility of the situation. Staff #1 stated she had asked Staff #3 for their fingerprint clearance card, but Staff #3 kept forgetting to bring it in to them. Staff #3 turned 18 yrs old in mid December 2015 and has no current fingerprint registration; 5 months from December 2015 to May 2016.
