Noncompliance Area: The seat on a blue chair in the school age room was cracked posing pinch points to the children in care.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The mat placement in the preschool 3 room lacked 2 feet of space on three sides of each mat while the children were napping.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: The child files #1, #2, #3, and #4 lacked the health insurance coverage and policy numbers on the emergency contact forms.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Child file #4, DOE 1/28/19, lacked an updated health report at least every 6 months for young toddler as required.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: Child files #4 and #5 lacked documentation of updated Influenza vaccines administered in accordance with the schedule recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The facility contained the following expired medications: Epi Pen, Inhaler, Methylphenidate liquid, Acetaminophen, and Children's Tylenol. Prescription Diazepam lacked the original prescription for the child in which the medication was intended.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: The toddler 1 room contained Mystasin cream that lacked a medication log for administration to the child while in care.

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: Staff file #3, DOH 9/30/19, contained a health assessment dated 1/7/20 and TB results dated 10/3/19, which were conducted after initial hire at the facility.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: Staff file #1, DOH 8/12/19, lacked copy of valid DHS FBI clearance in the file. Staff file #2, DOH 3/5/19, contained valid PA State Criminal Record dated 3/28/19, a valid Child Abuse Clearance dated 3/24/19, and a valid DHS FBI clearance dated 4/19/19, all of which were obtained after first date of working with children. Staff file #3, DOH 9/30/19, contained a DOE FBI which is not valid for child care and a receipt for NSOR clearance dated 12/3/19 which was after first date of working with children.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 and #3 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 and #3 may not work in a child care position at the facility.

Noncompliance Area: The preschool 3 room lacked a protective receptacle cover in the receptacle by the play kitchen on left side of the wall.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: The trash receptacles in the school age room, the preschool 4-5 room and the kitchen containing contaminated trash lacked lids.

Correction Required: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles.

Noncompliance Area: The first aid kit located in the preschool 4-5 room lacked non porous gloves.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: The girls room lacked a written handwashing sign posted near the toilet.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: On 8/21/19, in the preschool room staff #1 identified 17 preschool children applying naptime ratios when children were awake and active and not napping.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30.

Noncompliance Area: Staff #2 left the building during naptime leaving staff #1 with 17 preschool children during naptime on 8/21/19.

Correction Required: Staff persons who are on duty but are not providing child care during naptime shall remain in the child care portion of the facility premises.

Noncompliance Area: Staff person #1 did not wash a young toddler's hands after diapering the child before returning the child to the floor.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: Child file #1 lacked written consent signed by a parent on emergency contact form for emergency medical care prior to admission.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Noncompliance Area: Child file #2 lacked address of the release persons designated on the emergency contact form.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The facility had an Epipen on site for child #3 that lacked the written instructions for administration contained on the prescription label.

Correction Required: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable.

Noncompliance Area: Child file #1 lacked written consent signed by a parent on emergency contact form for administration of minor first aid procedures prior to admission.

Correction Required: A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: The director is scheduled in ratio and functioning as a group supervisor in the preschool room while more than 45 children enrolled at the center.

Correction Required: In a facility in which 45 or fewer children are enrolled, a director may also function as a group supervisor. An individual who functions in a dual capacity shall meet the responsibilities and qualifications of a director set forth in 3270.34(a) and (b).A director may not function as a group supervisor in a facility in which more than 45 children are enrolled.

Noncompliance Area: Specific children in the young toddler room were not using the mats that were assigned to that child.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Child file #1 and #2 lacked health insurance coverage and policy number on the emergency contact form.

Noncompliance Area: There was Barbasol shaving cream containing the statement 'keep out of reach of children' located in the preschool classrooms for use in crafts with the children in care.

Correction Required: Arts and crafts materials shall be nontoxic.

Noncompliance Area: The preschool 3 and preschool 4 and 5 classrooms lacked lidded trash cans for disposal of trash contaminated by human secretions.

Correction Required: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles.

Noncompliance Area: Child file #1 lacked copy child service report that was reviewed and signed by the parent.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Facility Person #1 may not work in a child care position at the facility.
