Child #1's on site immunization records did not show she had had the flu vaccine within the last 12 months and did not have a signed waiver on site. After checking with the family the child had received the flu vaccine but the family did not think to get another copy of the immunization records from the pediatrician. A copy of the current flu vaccine was obtained and emailed to the DHS Representative as well as a copy has been made and kept in the students file. In the future all immunization records will be checked at least twice a year, once at entry or return of the document and again at the 6 month update window to ensure all immunizations are up to date and on file.

Facility person #2's NSOR clearance had not returned from the state prior to waiver time expiring, as the program interpreted the waiver differently. The staff member was immediately removed from work and will remain on unpaid leave of absence until the NSOR clearance was returned. Once the clearance was returned the staff member able to return to work. In the future new hires must obtain their NSOR prior to being submitted for hire to our school board. Meaning all staff will have completed documents before their first day of work. In the meantime if any other staff member may have been hired under the waiver and they do not have the clearance back within the mandated amount of time the staff member will be removed and placed on unpaid leave until the clearance is obtained.

In the past the emergency management director, who provides the training signs the training sign in sheet, which is included in the inspection folder for the licensing rep to see, this year it was an oversight. We were able to secure the signature of the emergency management director on the sign in sheet and this was forwarded to the licensing rep. In the future the signature of the trainer will always be gathered on the staff sign in sheet.

MTSD Child Development Program employee's staff who meet the qualifications of a Site Director, however the School District's title for these individuals is Site Supervisor. Although the site supervisor had reviewed the evaluations with the MTSD Supervisor of Child Development and agreed with the evaluation information there was an oversight in getting the site supervisors signature on two of the program staff evaluations. The site supervisor signed the two evaluations in question. In the future, the MTSD Supervisor of Child Development will continue to work with the building site supervisor to complete evaluations and both signatures will be on the evaluation forms.
