Child #4 was enrolled for more than 60 days without an initial health report on file. The family was asked to turn in a current health report for the child. The family took the health report to the doctor to have it completed however our program then closed down for two weeks due to a high number of cases of Covid 19 within the school our program is located within (no confirmed cases in children enrolled in our program). The family will bring us a copy of the heath report on by 10/30/20. In the future health reports and immunization records should be received on site no later than 30 days from students enrollment date, or student will be suspended from the program until the heath report is received.

Documentation for the flu vaccination was missing from child 1, 2 and 3's records. The staff on site conferenced with the families to see if the students had received the flu vaccination with in the last year and encouraged the families to schedule an appointment for the vaccination, however, the three families choose to fill out flu exemption letters. In the future immunization records will be reviewed for any missing vaccinations. If vaccinations are missing the staff will council the families on the importance of vaccinations and encourage the families to schedule appointments to receive the appropriate immunizations, should a family choose not to immunize their child the staff will have the appropriate exemption letters completed by the families within the first 30 days of the child's enrollment.

We reached out to the parents of the children missing flu shot documentation and they will either give us the appropriate documentation or sign a letter stating their reason for their child not receiving the vaccine. We reached out to the parents to supply us with an immunization record for Child #4. In the future, we will monitor immunizations closely and get a letter of exemption from the doctor as to why child missed rotavirus during infancy.
