Child #1 did not have documentation of a flu vaccination with in the past year. The staff conferenced with the child's mother and encouraged her to schedule a flu vaccination for the child, however the parent choose to complete a flu exemption letter. In the future all student immunization records will be reviewed and families will be counseled on the importance of vaccinations and encouraged to schedule an appointment for any missing vaccinations or complete the appropriate vaccination exemption forms with in the first 30 days of enrollment.

In the future all immunizations will be checked within the first 30 days of school and if immunizations are missing the family will be asked for updates or letters at that time.

The staff Health Assessments were over 24 months old. In the future we will ask all staff health assessments be completed with in the 24 month requirement. The current assessments for these staff were current.

The parent had updated the form, but within a time frame that was outside of the 6 month window. In the future all families will update their forms at Pre-K Open house in August and again in January each school year to insure they are all updated in a timely fashion.

A bar of soap was added to the first-aid kit. In the future the kit will be checked several times throughout the school year to insure all items required are in the kit. We also keep extra items on site in a locked cabinet, so that the kit can be replenished when items are used.
