Findings: A site visit was conducted to discuss a bruise reported on the child while on site. The center owner stated no one at the site was aware of the bruises until cps showed up on the 24 of September 2024 and took pictures of the child leg. Discuss failure to report the incident of the bruising . Staff discussed the incident with the parent when the bruise was noticed on the child and a picture of the bruising was taken and shared with the parent but not the owner. Owner was not sure if the director at the time was aware of the incident.

Findings: LS conducted complaint investigation which is an addition to the complaint conducted on September 17, 2024. The center did not report the incident of the bruising on the child's leg because the staff saw the bruising and reported it to the parent. Staff took pictures of the bruising and sent to parent. Ls was informed that staff felt the bruising had taken place outside of the center. The center has a program on their system for staff to download any incidents that occur at the site. Although the staff reported the bruising to the parent , the staff failed to report the incident to the outside agency.
