Noncompliance Area: On 10/15/2020, observed a purple and pink table in the young toddler classroom with a loose leg, causing the table to be unstable. The table & chairs (which are intended for preschool age children) were also an inappropriate height for young toddlers who are unable to sit safely on the chairs with their feet touching the ground.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: On 10/14/2020, the refrigerator located in the infant classroom did not have a thermometer.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: On 10/14/2020, the older toddler classroom did not have a written plan of daily activities posted.

Correction Required: The written plan of daily activities shall be posted in the group space.

Noncompliance Area: On 10/15/2020, the agreement for for child #2 lacked the names of the persons designated by a parent to whom the child may be released.

Correction Required: An agreement shall specify the persons designated by a parent to whom the child may be released.

Noncompliance Area: On 10/15/2020, the emergency contact information for child #1 lacked the health insurance coverage information and policy number for the child.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: On 10/14/2020, the older toddler classroom did not have the written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency posted in a conspicuous location.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: On 10/15/2020, observed children eating lunch in the preschool classroom who had not washed their hands prior to sitting down. The children had used the bathrooms earlier, washed and hands, returned to the classroom and were playing in the classroom before sitting at the tables for lunch. The staff did not take the children back to the bathroom to wash their hands after playing with the toys.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: On 10/15/2020, observed tubs of spreadable cheese labeled, "Keep Refrigerated" in plastic bags on the counter of the kitchen storage area. The tubs of cheese were warm from sitting on the counter.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Noncompliance Area: On 10/12/2020, a copy of the menu was not posted at the facility or provided to each family at least 1 week in advance.

Correction Required: An operator shall conspicuously post the menu at least 1 week in advance or provide a menu to each family.

Noncompliance Area: On 10/15/2020, the file cabinet containing the children's records was unlocked. The file cabinet is located in the main entrance of the facility.

Correction Required: Child records are confidential and shall be stored in a locked cabinet.

Correction Required: Facility persons' records are confidential and shall be stored in a locked cabinet.

Noncompliance Area: On 10/15/2020, the files for staff person #1 and #2 contained disclosure statements signed by the new hires on 10/12/2020 which lacked the name and signature of a witness. The file for staff person #3, with an initial start date 04/12/2017, contained an NSOR clearance which was not completed before the 07/01/2020 deadline for existing staff; the NSOR clearance on file was dated 07/08/2020. The legal entity operates multiple locations and staff person #3 was working at location #1 prior to transferring to location #2 on 08/31/2020.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: On 10/15/2020, observed a diaper bag on the floor of the infant room with a bottle of Handsanitizer, Aveeno sunscreen, and Johnsons Lotion (labeled "Keep out of Reach of Children") in the side pocket which were accessible to the mobile infants and the school-age children in the classroom.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.
