At the time of the incident, the assistant director who was called to the room, inspected the pan in question, and discovered it had a sharp beveled edge that sliced the finger of child #1. The assistant director instructed the teachers to throw it away after the incident.

Our plan will be to view children's files monthly to ensure all children are in compliance with immunizations. Notifications will be sent to parents two months out to ensure all children's immunization records are up to date. Child #1 was dismissed from care and will not return until either the immunization is updated or the provides an exemption letter.

I will ensure that all medications that come to the center have prescriptions on them and in the original packaging. This eliminates the possibility of mistakes in administering medications to the wrong child.
