Description: No physical punishment

Noncompliance Area: A complaint investigation was conducted on 6/11/26. An incident occurred on 6/9/26. Through staff interviews it was determined that staff #1 slapped Child #1's hand as a form of discipline.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child. Tiered LIS: 1. A staff person may not use any form of physical punishment, including spanking a child. A staff person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. The date of correction for this portion of the plan must be immediate. 2. The legal entity must arrange for staff person #1 to receive a minimum of 3 hours of PQAS-approved training regarding appropriate discipline techniques. The legal entity must receive DHS approval of the training content prior to scheduling the training. Documentation of training will be kept in the staff's file. The operator shall provide a date for when this training will be completed.

Description: Clean and good repair/Small Toys and Objects

Noncompliance Area: A renewal inspection was conducted on 2/11/26. In the indoor play space, the green play mat squares were ripped on the corners exposing the stuffing inside. The inside stuffing is accessible to children that are still putting objects in their mouths. In the indoor play space, the large jungle gym has several tears in the protective mesh siding near the entrance of the jungle gym.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Description: Arrival/departure times/Designated release persons

Noncompliance Area: A renewal inspection was conducted on 2/11/26. Child #1's fee agreement did not specify the child's arrival and departure times and did not contain a release person.

Correction Required: An agreement shall specify the child's arrival and departure times. An agreement shall specify the persons designated by a parent to whom the child may be released.

Regulation: 3270.124(c)

Description: Each child care space

Noncompliance Area: A renewal inspection was conducted on 2/11/26. The facility uses the Brightwheel app in all the classrooms. The Brightwheel app's emergency contact section did not contain all the required DHS emergency contact information.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Description: Health information/Infant: updated health report every 6 months

Noncompliance Area: A renewal inspection was conducted on 2/11/26. Child #2's initial health assessment was not received within 60 days following the first day of attendance at the facility. This is evidenced by the initial health assessment of file dated 6/9/25. Child #6 has a health assessment on file dated 6/4/25 and does not have an updated health assessment on file. Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler.

Regulation: 3270.135(a)(3)

Description: Disposable diapers

Noncompliance Area: A renewal inspection was conducted on 2/11/26. In the Caterpillar classroom, the garbage can next to the changing table intended for diapers is not hands free. The foot pedal is broken.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Description: Emergency info/agreement updated 6 mos

Noncompliance Area: A renewal inspection was conducted on 2/11/26. Child(ren) #2-7's fee agreements have not been updated in the last 6 months. This is evidenced by the previous fee agreements on file dated: Child #2's 2/28/25; Child #3's 1/27/25; Child #4's 2/25/25; Child #5's 3/10/25; Child #6's 2/6/25; and Child #7's 2/20/25.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.
