Noncompliance Area: During an unannounced visit on 3/13/2019, it was verified that on 3/7/19 staff #1 was assigned a group of preschool children. The room that she was working in has a capacity of 25 and is used as one space for up to two groups of children. Staff # 1 was the only staff in the room and her group of ten preschool children were the only children in the room. The children were participating in open center time which allowed them to move to different stations throughout the room. While observing and directing some children from this group that were in front of her, she looked over at two children who were at a center to her left in a Plexiglas center. Staff #1 estimated that she had not directly looked at these children and this center for a period of time of no more than five minutes. Child #1, a male preschool child and child # 2, a female preschool child were within the Plexiglas cube station also referred to as a listening cube to the left of where staff #1 was facing. Walking closer to the cube Staff #1 observed the children in the cube, one child had pants down and the other child was inappropriately touching the child with the pants down. Also observed during the inspection on 3/13/19, child # 3 a pre-school child who was assigned to staff # 1 alone and asleep in a preschool room. Children and staff from this room and this child's group were observed having lunch in another room that is used for dinning. The child was left behind when the children and staff moved to the Dining room.

Correction Required: Children must be supervised at all times. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: Emergency Contact Form for Child # 1 did not contain health insurance policy number.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.
