Noncompliance Area: At the time of the inspection, the loose-fill rubber mulch surfacing under the embedded monkey bars, which had a fall height of 4'7", only measured 3 inches depth. The loose fill mulch surfacing under the embedded climber, which had a fall height of 4', only measured 4 inches in depth. Both of these pieces of embedded equipment require there to be at least 6 inches of loose-fill rubber mulch.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. Noncompliance Area: At the time of the inspection, in the Pre-K room, rest mats were observed to be torn and the inside foam exposed making them unable to be properly cleaned.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: At the time of the inspection, the agreement on file for child #6 was not updated every 6 months, as required. This is evidenced by the date of the latest signature of 8/1/25.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Description: Age and Training

Noncompliance Area: At the time of the inspection, staff person #5 only had 6 clock hours of childcare training during their 8/24-8/25 training period.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

Description: Fire safety - 1 yr.

Noncompliance Area: At the time of the inspection, staff files were reviewed. Staff did not complete fire safety training annually as required. This is evidenced by the current fire safety training date of 10/8/25 and the previous training date of 9/24/24.

Correction Required: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: At the time of the inspection, staff person #5's PA State Police clearance did not contain a dissemination date, and therefore, was not a completed clearance. Staff person #5 has had routine interaction and direct contact with children (see LIS code sheet).

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). . A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #5 may not work in a child care position with direct contact and routine interaction with children until a completed PA State Police clearance is on file.

Noncompliance Area: An allocated unannounced monitoring was conducted on 9/24/2025. At that time, the last documented fire drill was 7/11/2025, which is past 60 days. Director and staff reported that a drill was done but they forgot to document it.

Correction Required: The Director or designated staff person who is responsible for compliance with this chapter ensure fire drills are conducted at least once every 60 days.

Noncompliance Area: An allocated unannounced monitoring was conducted on 9/24/2025. At that time, the last documented fire detection system testing was 7/15/2025, which is past 30 days. Director and staff reported that a alarms have been tested but they forgot to document it.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.

Description: Services proceeded

Noncompliance Area: At the time of the inspection, the child service reports on file for child #1, #3, #4 and #6 were not updated every 6 months as required. This is evidenced by the current child service report dates of 1/2025 and the previous child service report dates of 5/2024.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information).

Noncompliance Area: At the time of the inspection, the health assessments on file for child #1 were not updated every 12 months as required. This is evidenced by the current health assessment date of 12/5/24 and the previous health assessment date of 12/7/22.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: File for staff person #1, who is currently employed as an aide, did not contain documentation of education, which at minimum, must verify completion of grade 8.

Correction Required: An aide shall have a minimum of an 8th grade education and 2 years of experience with children. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Description: Measurement and use of indoor child care space/Designated on facility schedule

Noncompliance Area: It was observed and confirmed by staff that on 6/23/23, Room 4, which has an established measured capacity not to exceed 8 children, had 12 children present from 9-9:20am in that child care space. The written daily schedule observed in Room 4 did not show that this child care space would be over capacity for a program activity during that time between 9-9:20am.

Correction Required: The capacity established for indoor space may not be exceeded except when older toddler, preschool or school-age children are participating in a program activity. When the number of older toddler, preschool or school-age children participating in a program activity exceeds the capacity of the child care space each time period shall be designated on the facility's schedule of daily activities.

Description: Director or designated staff person ensure compliance

Noncompliance Area: It was observed on the facility's fire drill log that the facility tested the fire alarms on 4/14/23; the next fire alarm test date logged was on 5/29/23. This was more than the 30 day fire alarm testing requirements established under section 1016(c) of the act (62 P.S. § 1016(c)).

Correction Required: The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.
