In the Preppers classroom the children did not have an sufficient quantity of books for the children. This was corrected during visit books were put in the classroom.

I observed that there were not identify pictures of the children on 4 of the emergency information forms.

Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. In space 8 three (3) medications did not have expiration dates on the written permission form. In space 9 three (3) medications did not have required information completed on the permission form.

For children under three years of age, materials were not made available to children on a daily basis. In space #8, we observed 10 wooden puzzles that did not have any pieces. When I ask the teacher about the pieces, she said they were in a plastic container on a shelf that were not accessible to the two-and-three-year-old children in the classroom. When she gave me the puzzle pieces, I started to complete the puzzles, five children immediately came over to me and began putting the puzzles together like it was a new activity. The teacher left the puzzle pieces on the shelf so the children could put them together. Toys and equipment must be accessible to the children in the classrooms.

All stationary equipment, more than 18 inches high, was not installed over protective surfacing. We observed on both outdoor playgrounds insufficient mulch for surfacing.
