Parent's medication authorization did not include required information. Medication authorization forms in 2 classes did not have the correct amount to be applied. This was corrected by the teacher during my visit.

An individual on a provisional criminal background status did not complete and submit documents that were specific to their former state of residence within the required time frame. Provisional expired on 12/11/23. Director was informed staff could not work until letter was updated.

For children under three years of age, materials were not offered in sufficient quantity to allow all children to use them at some time during the day and to allow for a range of choices. In the Preppers classroom the children did not have an sufficient quantity of books for the children. This was corrected during visit books were put in the classroom.

All walls and ceilings including doors and windows were not kept clean and in good repair. Paint on walls in some classroom needs to be touched up and some walls need to be wiped down and cleaned.

Parent's medication authorization did not include required information. Some medication forms did not have the amount or the amount was not a specific amount. This was corrected by the assistant administrator during our walk through.

Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. I observed that there were not identify pictures of the children on 4 of the emergency information forms.

The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff did not have this training on file. This was corrected during visit staff printed off training and put in file.

All walls and ceilings including doors and windows were not kept clean and in good repair. In classroom Preschool 3 walls need to be painted. Paint is peeling and missing in certain spots on the wall in the dramatic play area.
