Noncompliance Area: On Monday 10-14-19 at approximately 3pm, child #1 was left inside the facility in the Pre-Kindergarten room as the rest of the group transitioned outside. There were 2 staff with the children who were unaware that Child #1 was left inside until a parent picking up his child brought the child from the room to the playground. Child was unsupervised for approximately 10 minutes and in the primary care group of Staff #2

Correction Required: . The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training.

Director scheduled a 2 hour training on Supervision with Ms. Hall from Keystone STARS for all staff. Staff directly involved in incident completed an additional 3 hours through Better Kid Care centering on positioning, playground supervision and counting.

The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
