Noncompliance Area: An emergency contact person who was not a parent signed the six-month reviews for Child #1.

Correction Required: Following review, a parent shall attest to the accuracy of information in the emergency contact information and the financial agreement at each review by affixing a dated signature to the record.

Noncompliance Area: Staff #1 obtained 2.5 annual training hours during the anniversary year of 9/28/18- 9/28/19 and Staff #2 obtained 5.5 annual training hours during the anniversary year of 2/1/19- 2/1/20.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training.

Noncompliance Area: DHS FBI & PA child abuse clearance renewals for Staff #2 were obtained more than 60 months apart.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: Observed torn rest equipment with exposed foam.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The files of Children #1, #2, #3, #4, and #5 contained the original agreement. The facility did not make a copy for the file and give the original to the parent.

Correction Required: A parent shall receive the original agreement. The facility shall retain a copy of the agreement.

Noncompliance Area: The file of Child #2 lacked the complete work address of the enrolling parent on the emergency contact form.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.

Noncompliance Area: The files of Children #3, #4, and #5 lacked a reviewed and updated emergency contact form and financial agreement which should have been reviewed at least once in a 6-month period. Child #3 had an emergency contact form and financial agreement reviewed on 10/1/17, 4/10/18, and 11/2/18. Child #4 had an emergency contact form and financial agreement reviewed on 10/5/17, 4/10/18, and 11/5/18. Child #5 had an emergency contact form and financial agreement reviewed on 10/4/17, 4/10/18, and 11/2/18.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The facility lacked a posting of the telephone number of the nearest hospital, police department, fire department, ambulance, and poison control center by each telephone.

Correction Required: The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility.

Noncompliance Area: Observed broken and/or loose fence wire at the corner of the house and behind the flower bed in the outdoor play space.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: It was stated that a parent's request for Child #2, an infant, to sleep in a position other than on their back was honored due to a medical condition. Medical documentation had not been provided by the parent.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: Agreements for Child #3, 4 & 5 lacked the due date for fees.

Correction Required: An agreement shall specify the date on which the fee is to be paid.

Noncompliance Area: The Agreement for Child #4, a school-ager, included incorrect arrival and departure times.

Correction Required: An agreement shall specify the child`s arrival and departure times.

Noncompliance Area: The Emergency Contact Forms for Child #1, 2 & 5 lacked all of the addresses for individuals designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Two tubes of diaper cream were observed to have expired dates stamped onto the crimps (12/16 & 4/16).

Correction Required: Medication shall be stored in accordance with the manufacturer`s or health professional`s instructions on the original label.

Noncompliance Area: Staff #1 was observed to place a soiled diaper into a plastic bag, sit it on the floor beside the child being diapered and wait until the end of the diaper change to discard the bag.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.
