| Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 3/24/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. The following was discussed but not limited to: 1) Plexiglass barrier being added to diapering counter to allow for the lower adjoining counter to be used for food prep and other non-diapering items to be stored on the lower counter. Please email the Compliance Officer the following: 1) Current City of Gilbert fire inspection when obtained, and 2) Current certificate of insurance when it renews in April 2026. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. | |||

| Initial Comments: The purpose of the inspection was to conduct complaint case #000136149 investigation on 7/15/25 A full inspection was not conducted at this time. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Ratios observed were: Infants: 1:3, One-year-olds: 1:2, Two-three-year-olds: 1:4, Three-School age:1:4 and 1:4. There were 6 staff interviewed during this investigation. Others interviewed: The Compliance Officer attempted to contact the complainant twice and the Complainant did not respond. Documentation observed were rosters. Four staff files were reviewed and the fingerprint clearance cards for 3 of the 4 staff were verified to be valid per the DPS website. Note: One of the staff is not an adult and is not required to have a fingerprint clearance card. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that allegation #1 was substantiated. Allegations #2, #3 and #4 lacked sufficient evidence and were unable to be substantiated. The following was discussed, but not limited to: 1) Side-by-side occupied cribs are required to be spaced 24 inches apart if the crib does not have a non-porous barrier , 2) Mandated reporting requirements, and 3) Staff files are required to be maintained on premises (either electronically or a paper file) if staff from the other location is working at this location. | Initial Comments: No deficiencies were found at the time of the Initial License Monitoring Inspection conducted on 5/21/2025 and are subject to changes pending programmatic review. Additional cribs were also approved for use in the infant room at this inspection. The following was discussed but not limited to: 1) One of the 2 mats in the infant room is recommended not to be used for tummy time since the mat is too soft and the plastic coating is loose and puckering in areas, 2) Occupied cribs are required to be spaced at least 24 inches apart, 3) Designated exits can not be obstructed, and 4) Date of employment (versus start date) as it relates to staff obtaining fingerprint clearance cards, licensee verification of fingerprint clearance cards through DPS, orientation training completion, obtaining TB test etc. There was 1 new staff file reviewed.
