| Initial Comments: The purpose of the investigation was to conduct complaint #00182782 investigation on 09/10/2026. A focused inspection was conducted. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. DES Group Size was evaluated during the inspection. Ratios observed were: *Infants: 1:5 *Ones Room: 2:11 *Ones/Twos Room: 1:6 *Twos/Threes Room: 2:16 *Threes Room: 1:7 *Threes/Fours Room: 2:18 There were three staff interviewed during this investigation. The Compliance Officer contacted the complainant via telephone on 09/08/2026. Documentation observed was: *Incident reports *Parent Handbook There were no deficiencies found at the time of complaint #00182782 investigation conducted on 9/10/2026. Please submit the plan of correction in the LMS Portal within 10 days of receiving the statement of deficiencies. The fingerprint clearance cards for 19 of 20 staff members were verified to be valid, and 1 of 20 was determined to be not valid through the DPS website at the time of the inspection.
