Initial Comments: The purpose of the inspection was to investigate complaint #0174172 on May 19, 2026. A full inspection was not conducted at this time. The ratios observed were: Infants: 2:10 1's: 2:13, 3:13 2's: 2:9 3's: 1:13 4's/5's: 3:25 There were 4 staff interviewed during this investigation. Documentation reviewed: Incident Reports Complainant was contacted on 5/14/26. There were no deficiencies found at the time of complaint #0174172 investigation conducted on 5/14/26. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 1 of the 1 allegations lacked sufficient evidence to be substantiated
INSP-0164786 2025-12-11 Initial Monitoring Complete
Initial Comments: There were no deficiencies observed at the time of the Initial Monitoring Inspection conducted on 12/11/2025. A full inspection was not conducted at this time. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 1 of 1 fingerprint clearance card was verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1. Ensure 2 good-faith references for new staff are contacted. 2. Restroom maintenance and cleanliness.
INSP-0159978 2025-09-22 Compliance (Initial) Complete
Initial Comments: The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 09/22/2025, and the inspection is subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was given to the Provider at the beginning of the inspection. Please submit the plan of correction in the LMS Portal within 10 days of receiving the statement of deficiencies. The fingerprint clearance cards for 19 of 20 staff members were verified to be valid, and 1 of 20 was determined to be not valid through the DPS website at the time of the inspection. There were two vehicles approved for transportation (CK75066 and CL75065) During the exit interview, the following items were discussed, but are not limited to: *Infant feeding instructions will include when and how much to feed the infant *Exposed bolts at the base of the toilet will have a cap *Staff who drive the van will have current CPR/First Aid *Children who are one year old and walking will not be in the infant room *Staff file updates required when the new license is released
