The NSOR certificates on file for staff # 1, 2, 3, and 4 are all dated 8/13/20. The FBI clearance on file for staff #4 is not a DHS clearance and therefore is not valid.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff # 4 may not work in a child care position at the facility.

Noncompliance Area: There is a bike on the outdoor play area with a broken wheel.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Staff person # 3 started on 6/18/19. There is a state police clearance receipt dated 2/8/18 but no state police clearance. Staff person # 2 started on 10/1/18 and did not complete mandated reporter training until 5/6/2019.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

Noncompliance Area: There is a plastic police car with a hole in the plastic causing a sharp edge and pinch point in the outdoor play space. There is also a pink and purple car with a broken piece that has a pinch point in the outdoor play space.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children.A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Noncompliance Area: A COMPLAINT INVESTIGATION WAS HELD ON 8/1/18 REGARDING AN INJURY TO CHILD # 1. THE PARENT NOTIFIED THE OPEATOR THAT CHILD # 1 RECEIVED EMERGENCY MEDICAL CARE ON 7/30/2018 AS A RESULT OF THE INJURY. STAFF PERSON # 1 STATED THEY WERE UNAWARE THAT THEY NEEDED TO CONTACT THE REGIONAL OFFICE AND ADMITTED THE REGIONAL OFFICE WAS NOT CONTACTED WITHIN 24 HOURS.

Correction Required: The operator shall immediately notify a child's parent and shall telephone notice to the appropriate regional office within 24 hours if a child in care at the facility is hospitalized or receives emergency room treatment.

Noncompliance Area: TWO PACK & PLAYS IN THE INFANT ROOM ARE NOT LABELED FOR USE BY A SPECIFIC CHILD.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child`s parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: THERE IS LOTRIMIN & DESITIN IN THE INFANT ROOM THAT ARE NOT LABELED.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.
