Violation Observed: Based on information, ensure that adequate supervision is maintained at all times throughout the center. A named staff was changing diapers and failed to intervene when a named child attempted to climb a chair. The child then fell from the chair.

Violation Observed: Based on information, ensure that staff utilize positive methods of guidance and discipline at all times throughout the center. A named staff reportedly forcefully pushed a named child and the named child hit the back of his head against the wall.

Violation Observed: Ensure that the staff do not utilize any form of emotional punishment at any time in the center. At the time of the inspection a named staff was observed yelling "You have to hold still" at a named child during a diaper change in Toddlers B room.
