Findings: At the time of this inspection the LS observed activity plans for 19 children enrolled in the infant and toddler classrooms. The Fawns classroom has 1 that needs to be updated as well as the Hedgehogs classroom has 7 activity plans that are in need of their update. These reports are to be updated immediately. There are 2 children enrolled in Hedgehog classroom that are missing their plans all together. The LS reminded the facility that every child under the age of 2yrs must have a written individualized activity plan that is created with the parents and updated at least every 3 months. The facility is to have these collected and updated and submit evidence to the LS.

Findings: At the time of this inspection the LS observed one 2 month old asleep in a swing and a 5 month old asleep in a bouncer. The LS reminded the staff present that any child under the age of 12 months who falls asleep in a furnishing other than a crib, must immediately be moved to an approved sleeping arrangement. One child woke up while the LS was in the room, the staff moved the other child to their crib. The LS will conduct a follow up inspection(s) to ensure this is being maintained. The facility is to submit evidence all staff working with infants are aware of this requirement.

Findings: On September 29,2022 the office received an email regarding an incident where a teacher grabbed a child's chin and harshly told him to stop playing with his bento box. Two aides witnessed the incident and reported it to the administrators. The administrators reported the incident to other agency and OCC. The facility confirmed the teacher had not followed the discipline policy. The staff member is no longer employed at the facility.
