Description: Clean and good repair

Noncompliance Area: Observed peeling covers on the large green/orange and green blue blocks. Observed a peeling cover on the small blue block in the gross motor room. Observed a broken black toy bin in the gross motor room. In the school age room observed a damaged sensory table, broken white toy bin, the rest equipment shelf was not anchored, and exposed foam and peeling covers on 5 rest mats. Observed in the girl's restroom, a torn diaper changing pad and a dress up shelf which was not securely anchored. Observed a broken lego block in the skunk room. Observed in the squirrel room, a detached easel tray, loose legs on the tree mobile and a detached end on the wooden couch. In the racoon room observed a broken toy bin of little people. In the bunny room, observed a diaper changing table which was not securely anchored to the wall. In the fawn room observed a torn diaper changing pad. Observed staff cubbies in the front foyer area which were not securely anchored to the wall.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Clean, age appropriate

Noncompliance Area: Observed in the fawn room and in the fox room there were pack n plays which were not labeled with the name of a specific child.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Description: Infant sleep position

Noncompliance Area: Observed an infant sleeping in a bouncy chair in the infant room.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: Observed the agreement for child file #3 did not specify the child's date of admission.

Correction Required: An agreement shall specify the date of the child's admission.

Description: Written consent/Consent for administration of minor first-aid required prior to admission

Noncompliance Area: Observed the emergency contact form for child file #1 and #2 did not contain written parental consent for administration of minor first aid procedures by facility staff. Observed the emergency contact form for child file #2 did not contain written parental consent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Description: Insurance coverage information

Noncompliance Area: Observed the emergency contact form for child file #3 did not contain the health insurance policy number.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Observed the emergency contact information for child file #1 and #3 contained incomplete addresses for the person designated by the parent for the child to be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Description: Excursions

Noncompliance Area: Observed the complete emergency contact information for all of the school age children on an excursion to the park was not available to the staff on the portable tablet or when the group did not have a wifi signal.

Correction Required: When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion.

Description: Written emergency plan posted

Noncompliance Area: Observed the facility did not have a copy of the emergency transportation plan posted in each childcare and play space.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Description: Exemption documentation from parent/guardian

Noncompliance Area: Observed child file #2 did not contain an updated vaccination record for ongoing immunizations administered.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Description: Child's hands washed

Noncompliance Area: Observed a staff did not wash their hands after diapering a toddler in the toddler room.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Description: Disposable diapers

Noncompliance Area: Observed there was no hands free covered can at the diaper changing station in the fawn or the fox rooms.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: Observed staff file #1, 2, 5, and 7, who have already started working in childcare positions, did not contain initial health assessments. Observed staff file #8 contained an initial health assessment which was dated after their first day working in a childcare position.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Observed there was no infant feeding schedule written by the parent for infants receiving care at the facility.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.

Noncompliance Area: Observed a bottle which was not labeled with the child's name in the front infant room. Observed a bottle which was not labeled for the intended child in the fox room.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: Observed the facility was storing the child and staff files in a cabinet which did not lock.

Correction Required: Child records are confidential and shall be stored in a locked cabinet. Facility persons' records are confidential and shall be stored in a locked cabinet.

Description: Two written references

Noncompliance Area: Observed staff file #2, 4, and 9 did not contain the required 2 written non family references attesting to their ability to work in a childcare position.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.

Description: Availability of certificate of compliance and applicable regulations

Noncompliance Area: Observed the facility did not have a copy of the current certificate of compliance posted in a conspicuous location used by parents.

Correction Required: The facility's current certificate of compliance shall be posted in a conspicuous location used by parents. The operator shall provide the parent of each child enrolled with information on how to access the regulations in this chapter electronically and with instructions for contacting the appropriate regional child care office.

Description: Staff person - 18 yrs.

Noncompliance Area: Observed staff #6, who was listed as an aide on the staff data sheet, was under the age of 18 and would only qualify to be a volunteer.

Correction Required: A staff person shall be 18 years of age or older.

Regulation: 3270.32(a)

Description: Comply with CPSL

Noncompliance Area: Observed staff file #1, 2, 6, and 9 contained signed disclosure statements which were not signed by a witness. Observed staff file #3 contained a disclosure statement which was signed and dated more than 6 months prior to the staff starting in a childcare position at the facility. Observed staff file #1 did not contain a FBI fingerprint clearance or a NSOR verification letter. Observed staff file #2 contained a criminal background clearance receipt and not the actual clearance. Observed staff file #2 contained a child abuse, FBI fingerprint clearance, and a NSOR verification letter which were all dated after staff #2 began working in a childcare position at the facility. Observed staff file #9, who was working in a childcare position, did not contain a NSOR verification letter. Observed staff file #4, who was working in a childcare position, did not contain a child abuse clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # -1, 2, 3, 4, 6, 9- may not work in a childcare position at the facility.

Description: Locked or inaccessible

Noncompliance Area: Observed hand sanitizer, labeled keep out of the reach of children, accessible to children in the front foyer area. Observed an unlocked furnace room which allowed children to access spray paint, other toxic items, and cleaning supplies. Observed diaper cream, labeled keep out of the reach of children, accessible to children in low bins in the girl's bathroom used by children.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Description: Heat Source

Noncompliance Area: Observed a stove accessible to children due to the door of the kitchen/eating area was not locked.

Correction Required: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Regulation: 3270.75(d)

Description: On excursions

Noncompliance Area: Observed a group of children return from an excursion to the local park and the group of school age children did not have a first aid kit with them on the excursion.

Correction Required: One first-aid kit per child care group must accompany children and facility persons on excursions from the facility. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at §3270.75(c).

Description: Building Surfaces

Noncompliance Area: Observed the rug in the gross motor room was not securely attached to the floor creating a tripping hazard. Observed a spot of wet carpet next to the exterior wall in the gross motor room. Observed the ceiling vent in the boy's bathroom was not working correctly and making very loud scratching noises.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Regulation: 3270.77(a)

Description: No peeling paint or plaster

Noncompliance Area: Observed peeling paint on the door leading into the therapy room from the school age room.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Regulation: 3270.94(a)(1)

Description: Every 60 days

Noncompliance Area: Observed the facility had been in operation for more than 60 days and did not have documentation of completing a fire drill within the previous 60 days.

Correction Required: The Director or designated staff person who is responsible for compliance with this chapter ensure fire drills are conducted at least once every 60 days.

Description: Devices must be compliant/Director or designated staff person ensure compliance

Noncompliance Area: Observed the most recent documentation of conducting a fire alarm test revealed the last documented test was conducted on 2/13/26. Observed the fire alarms worked but were not operating as an interconnected system. Alarms were synchronized on the day of inspection and were then in operable interconnected condition.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.
