Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Observed staff file #2 contained updated FBI fingerprint clearance, child abuse clearance, and NSOR verification letter which were all dated more than 60 months after the previous valid clearance. Observed staff file #3 contained updated criminal background clearance, child abuse clearance, FBI fingerprint clearance, and the NSOR verification letter which were all dated more than 60 months after the previous valid clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Description: Designated on facility schedule

Noncompliance Area: Observed a group of 8 preschool children doing an art activity in a room with the capacity of 5, and the posted daily schedule did not document the time period when the room shall be over capacity for an established activity.

Correction Required: When the number of older toddler, preschool or school-age children participating in a program activity exceeds the capacity of the child care space each time period shall be designated on the facility's schedule of daily activities.

Noncompliance Area: In the outdoor play area, observed a piece of broken plastic fence, several places where the fence needed resecured to the fence post and zip ties cut off, and additional bricks were needed to finish the porch ramp. Observed in the upstairs room to the right, 2 exposed door hinges and a closet door which wouldn't shut creating a pinch point. Observed in the preschool room, 4 exposed screw heads in the bathroom and damaged wallpaper in the stuffed animal corner. Observed splintering wood on the base of a closet door in the infant room.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed 2 spots of damaged paint in the preschool room. Observed paint flakes on the floor of the front toddler room by the cabinets and on/by the windowsill.

Noncompliance Area: Observed several of the sinks throughout the facility did not have posted hand washing signs.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: Observed the front exit foyer was blocked with a cubby.

Noncompliance Area: Based on information obtained from staff and a review of child file #1, it was determined that the facility did not give the parent the original accident report on the day of the incident.

Noncompliance Area: Observed plastic cubby crates stacked in the main hallway which were a tipping hazard.

Noncompliance Area: Observed a spot of damaged drywall behind the door in the young toddler room.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed a broken toy tool bin in the preschool room. Observed several shelves in the toddler room which were not securely anchored. Observed a torn diaper changing pad in the toddler room.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Observed torn plastic wall covering, which was accessible to young children, in several places in the infant room. Observed loose staples close to the floor on the foyer wall. Observed torn plastic and tacks accessible to children in multiple spots and by the slide in the small child gross motor room.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: Observed a bottle in the infant refrigerator which was not labeled with the child's name.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: On 1/9/20, it was observed that three fire alarms in the lower portion of the facility, multipurpose room, kitchen, and gross motor room, were not working and did not sound an alarm when the system was tested.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Demonstration of Compliance with 34 Pa. Code 403.23 is required.

Noncompliance Area: Observed staff file #1 and #2, who have both been in a child care position for more than 90 days, did not contain documentation of the CPSL required mandated reporter training.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Staff will obtain documentation of completion of the mandated reporter training.

Noncompliance Area: Observed electrical outlets which did not have a protective receptacle cover on the Christmas tree in the foyer, in the multipurpose room next to the boys bathroom, and by the basketball hoop in the gross motor room.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Observed there was a missing ceiling tile in the foyer area. Observed there was a splintering rough edge on a closet door in the children's eating area. Observed a broken outlet cover plate by the slide in the small gross motor room along with a dislodged ceiling tile. .

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed in the fish room, several spots of damaged shelving with rough edges and pinch points. Observed a soft bunny with a torn seam and exposed lining in the young toddler room. Observed in the infant room a torn diaper changing pad and pack n plays with the mattress/bottoms not securely anchored with the manufacturer straps. Observed a diaper changing pad with a peeling cover in the girls bathroom. Observed a broken blue bin with rough edges in the multipurpose room.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Observed the emergency exit in the preschool room was blocked with a lego table.

Noncompliance Area: Observed at the time of arrival at the facility 2 staff with a mixed age group of 13 napping children. Staff #1 named the 4 children she was responsible for supervising. Staff #2 named the 6 children she was responsible for supervising. Neither staff stated they were responsible for supervising the last 3 toddlers in the room while the third staff was in another part of the building.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: Observed staff file #2 contained documentation of a bachelor's degree and 2 years experience with children, but did not contain documentation of the 30 credit hours in a human services field.

Noncompliance Area: Observed three cans of shaving cream labeled keep out of the reach of children, accessible to children on a low shelf in the art room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Observed a stove accessible to children due to an unlocked door and the knobs being on the stove.

Observed thumb tacks, with a diameter of less than 1 inch, used to hold up decorative paper accessible to children who are still placing objects in their mouths in the toddler room and the closing/end of the day room.

Noncompliance Area: Based on information obtained during the course of the investigation, through interviews and a review of facility documentation, it was determined that there was one staff with 14 school-age children on the afternoon of 11/21/17.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30.

Noncompliance Area: Observed at the time of the inspection the door to the kitchen/eating area was not locked and an operating crock pot with an exterior temperature of more than 110 degrees F was accessible to children on a low counter top.

Correction Required: Hot water pipes and other sources of heat exceeding 110 F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Noncompliance Area: Observed staff file #1 did not contain an initial health assessment prior to providing initial service in the child care setting.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Observed staff file #1, who has all three required valid clearances, did not have a signed disclosure statement in the file prior to providing initial service in the child care setting.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # -1- may not work in a child care position at the facility.

Noncompliance Area: Observed at the time of the inspection, one staff person present in the facility with 2 school-age children receiving care.

Correction Required: At least two facility persons shall be present in the facility when two or more children are in care. At a minimum, one of the facility persons shall be a staff person.

Noncompliance Area: Observed Rustoleum spray paint accessible to preschool and school-age children on a low cabinet in the unlocked main office.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Observed detached ceiling tiles in the gross motor room and in the entrance room. Observed peeling/damaged wallpaper in the foyer room. Observed damaged drywall by the steps to the multipurpose room and by the mirror in the preschool room. Observed splintering edges on the door trim by the door knob latch.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed in the art room a tall shelf which was not securely anchored. Observed in the fish room a damaged edge to a low shelf. Observed in the infant room a low shelf with splintering edges and a peeling cover on a recliner chair. Observed in the toddler room, a damaged gate with rough edges. Observed in the preschool room a torn couch cover with exposed foam and several torn pieces of the plastic wall decoration. Observed a piece of wood used as a shelf on top of several bins was not secured and could fall. Observed rest mat #42 has a peeling cover and exposed foam. Observed in the gross motor room a television stand which was not securely anchored to the wall. Observed in the toddler play area the ball pit had several damaged plastic balls and a see saw which was missing a handle.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Observed small macaroni, which had a diameter of less than 1 inch, located in the sand box used by the toddlers in the indoor play area. Observed a few low spots of loose plastic wall covering accessible to toddlers by the door of the todler room.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: Observed the high chair in the infant/toddler room did not have a safety strap.

Correction Required: High chairs shall have a wide base and a T-shaped safety strap.

Noncompliance Area: Observed the pack 'n plays at the facility were not labeled for the use of a specific child.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child`s parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Observed children's files #1, #2, #3, #4, and #5 contained incomplete original agreements.

Correction Required: A parent shall receive the original agreement. The facility shall retain a copy of the agreement.

Noncompliance Area: Observed there were no emergency contact forms present in the indoor play areas when the children were receiving care in these rooms.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Noncompliance Area: Observed child file #4, a young toddler, contained a most recent health assessment which was dated more than 6 months ago.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: Observed the facility did not have an established medication log for a bottle of Children's Advil which was stored at the facility.

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: Observed staff file #3 contained an initial health report which was dated approximately 2 months after staff #3 provided initial service in the child care setting.

Noncompliance Area: Observed staff files #2 and #4 contained documentation of initial training in the emergency plan completed more than 1 week after providing initial service in the child care setting.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.
