Findings: Based on facility documentation, staff interview and interview with the Director, it was determined that on 11/28/2018, Child #1 had left the 4's Room through a door to a fenced area outside the room on the east side of the facility for 2 minutes with no staff member. There was 1 child with no staff member during this time.

Findings: Per allegation, based on interview with the Director, Staff Statements, and a review of facility documentation, Surveyor determined that Child #1's health and safety was placed at risk of harm when Child #1 left the 4's Room alone through a door to an outside fenced area on the east side of the facility on 11/28/2018 from 6:07 PM to 6:09 PM.

It’s funny because the Sunrise I used was closer to hmm say 97th? 92nd? Ave … so it really makes me wonder if that professional, monitored facility had the stupidity to pawn my children off to an in home daycare to cut corners and make more money. If so … I’d have to say … RUN.
