A paper copy of the Notice of Inspections Rights was given to the facility at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The DES Group Size requirements were observed in compliance at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: *Medication written authorization form includes the prescription number if applicable *A Facility Representative will review and sign the staff member's TB Attestation *Staff Files will include a copy of the back of the fingerprint clearance card *The Emergency Information and Immunization Record Card includes an email address
