In Space 1, there was one (1) diaper cream with no name of the child and another one with no name of the product on the permission to administer forms. One diaper cream with no amount listed, one child with 'as needed' under amount. There was another permission to administer form with no amount listed. | Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. In Space 2 there were three (3) bottles not dated. This violation was corrected during the visit today. | |||

I observed Catch Master glue boards in an unlocked lower cabinet in Space 4. There were Clorox wipes, Zep towels, and hand sanitizer in unlocked cabinets. In Space 3 I observed Gold Bond softening cream, Gold Bond foot cream, hand sanitizer, tear-free shampoo, and Dawn dish soap in unlocked cabinets. | |||
