Description: Indoor equipment was not provided to allow each child a choice of at least 3 activities. All of the toys/equipment is kept in a back bedroom or in the breeze way which is not used during the winter due to it not being heat, and therefore not accessible to the children. At the time of the visit there were several books, two puzzles, and a bucket of plastic Easter eggs in the living room for 6 children.

Description: At the time of the visit a 2 year old child was observed sitting in floor a high chair, with the tray on watching TV for approximately 15 min. The child was not eating any food. Because the tray was on, it restricted the child's movement from being able to get up and walk around freely. As soon as the provider took the tray off the child got out of the seat.

Description: At the time of the visit Staff A and Staff B did not have any documentation to show they met the continuing education requirement. Staff B only has 10.5 out of the required 15 hours of documented continuing education for 2018.
