The file for staff person #2, whom was hired as an assistant group supervisor on 3/22/19, lacked an FBI Clearance that was completed prior to her first day working with children. The FBI Clearance in the file was dated 9/11/19.

A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

The emergency plan training for staff person #1 was completed on 6/4/18 and 7/17/19. The emergency plan training for staff person #2 was completed on 6/1/18 and 7/17/19. The emergency plan training for staff person #3 was completed on 7/12/18 and 8/9/19.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: The file for staff person #4, whom was hired provisionally on 1/3/19, lacked a valid Child Abuse Clearance and FBI Clearance. The Child Abuse Clearance was requested for volunteer purposes instead of employment purposes. The FBI Clearance was processed through the Department of Justice instead of the Department of Human Services. The file for staff person #5, whom was hired provisionally on 9/14/18, lacked valid State Police and Child Abuse Clearances. The State Police and Child Abuse Clearances were requested for the purpose of volunteer instead of employment purposes. The State Police Clearance was dated 3/9/17 and was updated on 4/19/19. The Child Abuse Clearance was dated 3/17/17 and was updated on 4/19/19.

Noncompliance Area: According to staff statement if a bite does not break the skin they do write up a boo-boo report to give to the parent instead of a incident report. However, a copy of the report is not kept in the file nor placed in the incident file each time a child gets bitten.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Noncompliance Area: The file for child #1 lacked an emergency contact form that included the insurance coverage policy number.
