Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. The television was on with children under the age of three in care.

Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. In the bathroom a small bottle of cologne was on the sink, in the living room space, a bottle of scentsy odor sanitizing spray was stored below five feet. In the kitchen a can of Lysol spray was stored above five feet but not locked. On the van in the driver door was Bath and Body Works Hand cream, a bottle of Excedrin, Cherry Air freshner spray, Carmex, Poo Pouriri Toilet Spray and Purell hand sanitizer.

Individuals who provide care for 5 hours or more a week, during planned absences of the operator, had not completed a health questionnaire. The additional caregiver did not have a health questionnaire on file.

The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. Documented shelter-in-place drills were not conducted every three months, the last recorded drill was 2/1/2022.

The operator did not review the FCCH EPR Plan with additional caregivers on an annual basis. The EPR was not reviewed annually with the additional caregiver hired in 2020.
