|Findings: Based on Surveyor's observation, an inhaler and an epipen were found stored in a medication lock box that had the key placed in the lock in room 4B.|
|Findings: Based on Surveyor's observation, the green and brown Little Tykes toddler slide was not anchored to the ground and it was wobbling as the children were climbing on it.|
|Findings: *Room #2A- The Surveyor observed that the last names of 3 of the children were not included on the roster; *Room #2B- The Surveyor observed 8 children present in the room at the time of the inspection but the roster had the names of only 7 children listed as present; *Room #4A-The Surveyor observed 13 children present in the room at the time of the inspection but the roster had the names of 16 children listed as present; *Room #4B- The Surveyor observed 16 children present in the room at the time of the inspection but the roster had the names of 18 children listed as present; *Room #4C- The Surveyor observed 1 child present in the room at the time of the inspection but the roster had the names of 15 children listed as present. The Teacher stated that the 14 other children listed had left for the day; and *Room 3A- The Surveyor observed 9 children present in the room at the time of the inspection but the roster had the names of 15 children listed as present.|
|Findings: The Surveyor observed that the lesson plans posted in Rooms #4B and #3B did not specify a large muscle development activity for each day of the week.|
|Findings: The Surveyor observed the following during the inspection of the diaper changing areas: *Room 2A- The soiled clothing container was stored in the bathroom and not in the diapering area, as required; and *Room 2B- The soiled clothing container was cloth and not waterproof and sanitizable. The container was also not equipped with a lid.|
|Findings: The Surveyor observed the soiled diaper containers in Rooms 2A and 2B were stored on the floor next to the diapering tables and were accessible to the children in the room.|
|Findings: The Surveyor observed Epipens stored in first aid kits that were mounted on a wall in room #2B, #4A and #4B. The keys to the first aid kits were stored in the lock of the first aid kits. The Epipens in these room were accessible to the children.|
