Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 3/26/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. The following was discussed but not limited to: 1) Anniversary fee due in April 2026, 2) All current staff are required to submit DCS clearance through the CBC portal that was effective July 2024, and 3) New rules effective 8/2025 and forms available on the Department’s website. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.
INSP-0107864 2025-03-26 Compliance (Annual) Complete
Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 3/26/2025 and are subject to changes pending programmatic review. The following was discussed, but not limited to: 1) Items stored in a bathroom are required to be stored inaccessible to children and stored in either a closed cabinet or on a shelf in a covered, waterproof container, 2) Bare wood on the 2 outside garden containers are required to be sealed before the wood starts to splinter, 3) Anniversary fee due in May 2025, 4) Submit the approved City of Scottsdale fire inspection when it is obtained, 5) Informing parents of children who have a religious or medical exemption for the child’s immunizations, the rule requirements related to the child not being allowed to attend during a communicable disease outbreak. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 6 staff files reviewed. 6 of the 6 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was emailed to the Provider at the time of the inspection.
INSP-0042680 2024-04-24 Compliance (Annual) Complete
Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 4/25/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 5 of 5 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Diffusers in the classrooms. 2. Update the emergency, accident and evacuation plan. 3. Staff training hours. 4. Defrosting the freezers. 5. The exit doors remaining unobstructed during hours of operation. 6. Children's personal products labeled with a child's first and last name. 7. Children's Record cards must be complete.
