| A safe indoor and outdoor environment was not provided for the children. Landscaping rocks were observed along the fence line on the infant playground accessible to children posing a choking hazard. Plywood gates were observed at each entry point of the playgrounds. It was explained that the metal gates were being powdered coated and they were waiting for installation. The plywood was splintered on the edges causing a potential hazard to children. Screws were observed poking through the plywood into the play area. | |||

Painters were onsite today. An open bucket of paint, caulk, tube of epoxy, and a metal scraper were observed unattended on the floor of Space 12.
