|Findings: Based on a review of staff #3's file (date of employment 2/26/2019), the Surveyor determined that the file contained documentation indicating the staff's freedom from infectious active tuberculosis that was completed on 2/27/2019, rather than on or before the staff's starting date of employment.|
|Findings: Based on a review of the file for staff #4 (a Preschool teacher), the file contained documentation of the staff's education from a foreign country (Colombia). The level of the staff's education had not been evaluated for the education's equivalency and it could not be determined if the staff met the minimum qualifications of a teacher caregiver.|
|Findings: Based on the Surveyor's review of the file for staff #4, the file contained documentation indicating that the staff completed only 16 of the required 18 hours of training for the period of 8/20/2018 through 8/20/2019.|
|Findings: Based on Surveyor's inspection of the diapering changing table in Room #4, the Surveyor observed two small holes on the table where the plastic tabs covers had been removed.|
|Findings: Based on Surveyor's observation, the following water temperatures were observed at the diaper changing sinks: *Room #1- 75 degrees F; *Room #2- 73 degrees F; *Room #4- 78 degrees F; and *Room #12- 80 degrees F.|
|Findings: Based on Surveyor's observation, the soiled diaper container in Room #12 did not have a waterproof liner in it.|
|Findings: Based on the Surveyor's review of the June 2020 menu, the menu twice stated "Tropical Fruit" and twice stated "Fruit Cocktail" but did not list the specific fruit and the components of the fruit cocktail.|
|Findings: Based on the Surveyor's observation, the plumbing fixtures were not maintained in a clean and working condition in the following areas: *Room #4 - There was a brown ring inside the 2 toilets at the water line; and *Room #8 - One of the toilets was unflushed and contained urine and wood chips.|
|Findings: Based on the Surveyor's observation, a child's two pack Epipen box only contained 1 of the 2 Epipens. The Director stated that the second Epipen had not been brought in and acknowledged that the did not have written permission from the child's medical physician that allows the Epipens to be separated.|
|Findings: Based on the Surveyor's observation, the wood chips measured the following depths: Toddler Playground *Less than 3 of the required 6 inches at the base of the pink slide; and Preschool Playground: *Less than 3 of the required 6 inches at the bases of the 2 beige slides; and * Less than 3 of the required 6 inches under some of the swings.|
|Findings: Based on Surveyor's observation, 14 children were present in Room #8 but the roster had the names of 15 children listed as present.|
|Findings: Based on the Surveyor's review, it was determined the City of Scottsdale fire inspection completed on 6/5/19 had 1 violation and the fire inspection was not approved.|
|Findings: Based on Surveyor's observation, none of the 11 infants' written feeding instructions were posted in the kitchen.|
|Findings: Based on Surveyor's observation, the following items stored on the diaper changing area: *Room #2- A rag was stored on the diapering counter; *Room #11- A roll of plastic bags was stored on the diapering counter; and *Room #12- A Lego toy was stored on the diapering counter.|
|Findings: Based on Surveyor's observation, the soiled diaper container in Room #12 was not equipped with a waterproof liner.|
