Please submit the Plan of Corrections via the portal within 10 days. ***The Director was reminded of the license expiration and to log on to the portal to pay the fees prior to the license expiring. 7 of 7 Fingerprint Clearance Cards reviewed were valid via the DPS website. The Empower self-evaluation was emailed at the time of the inspection. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. The following was discussed but not limited to: 1. Staff files must be complete with an emergency contact name and phone number. 2. 10 day new staff training must be signed by the Director. 3. All chemicals and toxics must be inaccessible to enrolled children. 4. Toilet room exhaust fans must be in operable condition. 5. Continue to monitor the rust on the playground fence. 6. The police report regarding the incident and the licnesee’s internal investigation documentation were also reviewed.
