We, the Catholic educators of SS. Simon & Jude School, recognize parents as the primary educators in the formation and education of their children. We will work in partnership with parents to build upon this foundation. We believe in our God-given mission to facilitate the growth of each child in this faith community as a whole person, with responsibilities to God, the global community, and all creation. We strive to provide each student with the opportunity to acquire a personal relationship with Jesus in an atmosphere alive with the Spirit of God.

We emphasize the importance of individual potential, academic achievement, intellectual, and aesthetic growth. Believing in the uniqueness and dignity of each individual, we encourage each student to develop a strong, positive self-image by providing and expecting a Christian atmosphere characterized by understanding, respect, and acceptance of all people. We also believe every child is capable of developing moral values and self-discipline skills.

We are committed to teach and model behavior which will enable our students to develop the skills to become self-disciplined, reflective, discerning Catholic Christians. We will assist in the formation of children as they grow in Freedom, Sincerity, Truthfulness, and Justice. We expect our students will find God in each life experience and go into the future embracing the honor and service of God.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 1/14/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Designee at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***The Director was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Staff files must be complete with all of the required documentation. 2. Spacing of children's nap mats during naptime. 3. Please remove the liability waiver statement. 4. Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 1/17/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but no limited to: *The licensee will redo Staff #4's new staff orientation form that was completed when the staff was hired in August 2016. (The licensee could not locate the original form that was completed in 2016). There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Program Assessment was completed at the time of the inspection.

Instead, the records for children #1, #2, #3 and #4 contained the parent's initials or an illegible squiggle mark on various days throughout the month of January 2022.

Initial Comments: Based on the Surveyor's observation, the facility was not maintained in a clean condition in the following areas: *Boy's breezeway bathroom: 2 tiles were missing under 2 of the urinals and the remaining holes had a buildup of brown residue on the floor area.
