The substitution of Pepperoni pizza and apples instead of Sloppy Joe's and peaches was not documented on the menu.

Findings: Based on the Team Leader and the Surveyor's observations and information provided by the staff at the time of the inspection, it was determined that the required ratios were not maintained in the following classrooms: * School-Age classroom: Upon the Team Leader's and the Surveyor's arrival at the facility at 12:51 pm, the staff in the School-age class walked out of the classroom and went into the 3's/4's classroom leaving the School-age classroom at a 0:6 ratio instead of the required ratio of 1:6. * Infant classroom: The ratio was 0:1 in the infant classroom instead of the required ratio of 1:1. * 3's/4's classroom: The ratio was 1:17 instead of the required ratio of 2:17. The director designee stated that the staff had just gone on break. * This was previously cited on 8/13/2020, 2/6/2019, 12/12/2018 and 7/19/2018.
