|Findings: The caller states that the provider is gone often more than 2 hours and that parents aren't notified of the sub use. The provider did not have a record of substitute use. The provider did have an electronic calendar and upon reviewing that the provider was able to obtain the dates and approximate times that the additional adult was alone with at least 1 FCC child. Please keep a clear written record of any substitute use more than 2 hours-the date, the time frame, the ages and number of children present and who was in charge of the children. The provider recreated a record on site based on her electronic calendar.|
|Findings: There is a new 6 year old who started last week, and there is no emergency form present for this new child. Please immediately obtain a complete emergency form for this child and forward written corrective action within 30 days.|
|Findings: The provider's contract notifies parents that she has substitutes that may be in charge of the children. The provider also reports that she has verbally notified parents of most sub use. The provider doesn't have a written record of advance notification of sub use. The provider checked her electronic calendar for sub use and there have been several days that a substitute has been alone with the children for more than 2 hours. Please immediately create a plan and a written way to notify parents of substitute use, from now on. Please forward written corrective action within 30 days.|
|Findings: The provider does have 4 approved nap sources for children under age 12 mos. However, upon inspection a 9 month old baby was observed sleeping in an infant seat. The provider allowed the infant to continue to sleep in the infant seat for about an hour; until the baby woke up and joined the group. If an infant falls asleep anywhere besides an approved sleep source the baby should be moved to an approved sleep source. Please in the future be sure that infants are sleeping in approved sleep sources, within sight and sound of the provider.|
|Findings: The provider's 1st aid and CPR expired in March. The provider is reportedly working on scheduling a class to complete 1st aid & CPR ASAP. Please forward a copy of the current first aid and CPR cards upon completion. Please make arrangements to complete 1st aide and CPR immediately.|
|Findings: Provider has been napping the infants in the main floor office and she has remained on the main floor to do some house chores such as laundry and food prep and kitchen clean up. Provider states she was doing 15 minute checks on the infants. Provider misunderstood the regulation. Reviewed that the provider must be within sight AND sound of the infants when they are resting at all times. The provider or additional adult would need to stay in the room with the infants. Provider wasn't using a video and sound monitor. The provider does have 1 video and sound monitor that she states she was using for the older napping children left in the basement. Provider states that she or her additional adult were maintaining 15 minute checks on the older children in the basement. Provider states they didn't leave the older children in the basement, to go upstairs to nap the infants, until they were asleep. Today, the provider, napped one infant in the basement and the additional adult stayed in the room upstairs with the other 2 napping infants. Discussed ways to meet the regs. The provider started napping the infants upstairs sometime after Lic.Spec. last inspection. Please forward your plan.|
