Date Type Regulations Status
2023-11-01 Mandatory Review 13A.15.03.05C(1) Corrected
Findings: The provider has another grandchild residing in the home who was born in March 2023. The provider did obtain a medical report for the grandchild and a copy was provided to the licensing specialist. The provider's capacity will be reduced to 4 total children because there are now 4 residents under the age of six.
2022-12-12 Full 13A.15.03.02A Corrected
Findings: The provider was unable to locate the health inventory (includes individual needs and lead) for BC. BC did have immunizations on file but they need to be updated to show the current immunizations for her age. JL needs documentation of his MMR/varicella vaccinations as these were not on his immunization record.
2022-12-12 Full 13A.15.03.03B Corrected
Findings: The provider will ensure that attendance is maintained on a daily basis for all children enrolled. Current attendance (Nov/Dec) was not available for viewing.
2022-12-12 Full 13A.15.03.03E Corrected
Findings: The children in care need documentation of the consumer pamphlet in their file or proof the parents were told how to obtain the document. A copy of the consumer pamphlet was given to the provider to copy and give to the parents.
2022-12-12 Full 13A.15.03.03F Corrected
Findings: The provider has documented all fire drills for 2022 with the exception of November and this month. The provider will ensure each fire drill is documented monthly and the disaster drills are also documented.
2022-12-12 Full 13A.15.03.04A(1) Corrected
Findings: JL does not have a completed emergency card on file. Please have his parents complete the form, submit a copy to the licensing specialist then keep it in his file.
2022-12-12 Full 13A.15.03.05C(1) Corrected
Findings: The provider has another grandchild living in the home, ST, who was born on 6/14/2021. The licensing specialist was just made aware of the child today at the unannounced full inspection. The child has a doctor's appointment on 11/14/2022 and will have a medical completed at that time. Please forward the completed medical to the licensing specialist.
2022-12-12 Full 13A.15.05.04B(2) Corrected
Findings: The provider must ensure there is a gate at the bottom of the stairs by the front door so children do not have access to the stairs going to the second level of the home. The gate was not in place at the time of the inspection.
2022-12-12 Full 13A.15.10.02 Corrected
Findings: The kitchen and dining area are specified as off-limits on the provider's registration. The barrier for that area was not in place at the time of the inspection. There were disinfectant sprays on the counter and drawers without fronts open to the children. The provider must ensure these items are kept inaccessible to the children and that the barrier to the dining/kitchen area is in place whenever children are in care. The children do have to go through this area to use the bathroom and should always be supervised by the provider when going to the bathroom.

2019-01-23 Conversion 13A.15.04.03B Corrected

When the Licensing Specialist telephoned the provider to schedule the conversion inspection and the Infant/Toddler 4 under 2 years old orientation, the provider admitted that both the family child care children under 2 years old were in care. The provider also admitted during the telephone conversation that her grandson, who is also under 2 years old, was present with his mother.
