Noncompliance Area: The file for Staff #2, a provisional hire, contained a state police clearance dated after the first day of employment in a child care position and a receipt for state police clearance which lacked a date. The file for Staff #2 also contained an application for NSOR dated prior to employment, and an NSOR clearance dated after the provisional hire period had expired. Staff #2 was not removed from a child care position at the end of the provisional hire period.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
