The week of 1/13/26 to 1/16/26 there were 10 children signed in at one time each day. The week of 1/26/26 to 1/30/26 there are 10 to 12 children signed in at one time each day. The week of 4/6/27 to 4/10/26 there are 9 children signed in at one time each day. On 3/2/26, 3/4/26, 5/11/26 there are 9 children signed in at one time. On 3/9/26 and 3/9/26 there are 10 children signed in at one time. Violation

2026-05-28 DCF 250.11(2)(g) states in part, a person cannot hire an employee within 2 years from the date the employee had a child care license revoked. Text messages show that an individual who was revoked within the last two years is working and providing care at the center. Licensee would have been aware of the revocation, as the licensee was a household member when the individual's center was revoked. Violation

2026-05-28 Licensee did not provide current and accurate information to the department. During a complaint investigation for an injury to a child the licensee reported a certain individual does not work at the childcare however information from a parent along with text messages and attendance records show the individual is proving care. Licensee said the center does not transport. Information from a parent and text messages show transportation was provided. Licensee said parent picked up a child on 5/19/26 at the center, documentation shows the child was picked up at an alternate location. Violation

During the investigation parent stated the child does not attend childcare on Wednesdays. Out of 8 weeks of attendance reviewed the child was signed in and out of care on 7 Wednesdays. Violation
