Noncompliance Area: A complaint inspection was conducted 6/8/26. During the investigation, it was discovered that on 5/28/26 Child #2 was left on the playground. Video footage showed that Child #2 was left on the playground from 11:53-11:57am with another staff and group of children before being brought back to their classroom by another staff. Child #2 was in the primary care group of Staff Person #2. Staff Person #2 was terminated from employment at the facility on 6/1/26. Through staff interviews, it was discovered, that a lapse of supervision occurred on 6/19/26. Video footage showed Child #2 and Child #3, both young toddlers, attempting to open the gate to the playground fencing and eventually succeeding. The children then began walking down the sidewalk outside the playground. This incident was unnoticed by Staff Person #3. The children were outside of the playground fence for less than a minute before being discovered.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. The training must be PQAS approved, in-person, and outside of childcare hours. The legal entity must receive DHS approval of the training content prior to scheduling the training. The operator shall provide a correction date for when the training and follow up with Quality Coach will be completed. 3. The facility will demonstrate compliance with 3270.113(a) and 3270.113(a)(1) for a period of three months. There may be no instances of noncompliance during this time period during unannounced inspections. The operator shall provide a correction date that is 3 months from the date of plan submission.

Description: No physical punishment

Noncompliance Area: A complaint inspection was conducted 6/8/26. During the investigation, video footage of Room 17 was reviewed from 6/4/26 at 2:47pm. At that time, Child #1 is seen banging a book on a shelf. Staff Person #1 leans over the shelf and roughly jerks the child up and then back down to the floor before blocking the view of the camera. Staff interviews discovered that while blocking the camera, Staff Person #1 had pushed the child down again, grabbing the child by the wrist and then scolding the child, Staff Person #1 is heard in the video saying "All done, no thank you. Your friend is sleeping. You are not being nice. No" Child #1 crawls away toward another shelf. Staff Person #1 walks toward Child #1 and leaning over the next shelf and pushes the child roughly by their head. Staff Person #1 was terminated from employment at the facility on 6/5/26.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

Description: Small Toys and Objects

Noncompliance Area: A renewal inspection was conducted 5/6/26 and 5/8/26. On 5/6/26, plastic bags were observed in a floor level cubby and diapers in plastic packaging were observed in the unlocked drawers next to the changing table of Room 17, where they were accessible to the mobile infants receiving care at that time.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Through interviews, it was found that Staff Person #1 was discovered sleeping in Room 1 by Staff Person #2, who had to nudge and call the staff's name to wake them. Staff Person #2 stated that at least five of the nine young toddlers in the room were awake at that time. This occurred on 4/29/26. Staff Person #1 was immediately terminated from employment on 4/29/26.

Noncompliance Area: A complaint inspection was conducted 5/6/26 - 5/8/26. On 5/8/26, Staff Person #6 was observed grabbing a toy from the hands of Child #5 and tossing it aside while saying "Okay, you're done!" Staff Person #6 then roughly placed Child #5 in a chair. Staff Person #6 then picked up Child #6 and roughly placed that child into a chair, pushing the chair up to the table roughly enough that the child's head bounced back.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child. TIERED LIS 1. The legal entity must arrange for Staff Person #6 to complete the training entitled "Big Emotions and Self-Regulation for Adults and Children" through Better Kid Care. Noncompliance Area: A renewal inspection was conducted 5/6/26 and 5/8/26. On 5/6/26, two unlabeled bottles were observed in the refrigerator of Room 4.

Noncompliance Area: A renewal inspection was conducted 5/6/26 and 5/8/26. On 5/8/26, the staff files were reviewed. The file for Staff Person #3, Staff Person #4, and Staff Person #5 included emergency plan trainings for the staff showing that the plan had not been reviewed within twelve months of the previous training. This was evidenced by the emergency plan training dates of 7/31/24 and 2/16/26 for Staff Person #3; 9/5/24 and 2/16/26 for Staff Person #4; and 9/5/24 and 2/16/26 for Staff Person #5.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: A renewal inspection was conducted 5/6/26 and 5/8/26. On 5/8/26, the staff files were reviewed. The file for Staff Person #3, Staff Person #4, and Staff Person #5 included fire safety trainings for the staff showing that the trainings had not been completed within twelve months of the previous training. This was evidenced by the fire safety training dates of 7/25/24 and 2/16/26 for Staff Person #3; 6/19/24 and 2/16/26 for Staff Person #4; and 9/16/24 and 2/16/26 for Staff Person #5.

Correction Required: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: A renewal inspection was conducted 5/6/26 and 5/8/26. On 5/8/26, the staff files were reviewed. The file for Staff Person #4 contained a PA Child Abuse Clearance and FBI Clearance that were not updated within 60 months. This is evidenced by the PA Child Abuse Clearances on file dated 1/29/20 and 2/14/25 and the FBI Clearances on file dated 1/22/20 and 3/3/25.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: A renewal inspection was conducted 5/6/26 and 5/8/26. On 5/6/26, Pipette sunscreen was observed in an open diaper bag of a floor level cubby in Room 1, the young toddler room. The sunscreen was labeled "keep out of reach of children."

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: A renewal inspection was conducted 5/6/26 and 5/8/26. On 5/6/26, the first aid kit in Room 7 did not include bandages and the first aid kit in Room 4 did not include tweezers.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: A renewal inspection was conducted 5/6/26 and 5/8/26. On 5/6/26, the bathroom area of Room 1 was observed to have toys scattered all over the floor, blocking access to the sink used for handwashing after the young toddlers are diapered.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
