Initial Comments: The following deficiencies were observed the time of the annual inspection conducted on 03.04.26, and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the facility director at the time of the inspection. Two fingerprint clearance cards were reviewed. One fingerprint clearance card was verified as valid through the DPS website. The following was discussed, but not limited to: *Ensure gauze is stored in a sealed sterile container. *Ensure staff files are complete. *Ensure trash container is maintained in a clean condition. *Ensure dust pan is not stored near drinking water containers. *Ensure rugs are maintained in a clean condition. *Ensure the outdoor activity area is maintained in a clean condition and free from hazards. *Ensure the freezer thermometer is maintained below zero degrees. *Ensure children's Sign In/Out are complete.

Initial Comments: The following deficiencies were observed at the time of the compliance inspection conducted on 3/18/25 and are subject to changes pending programmatic review. 4 of 4 Fingerprint clearance cards reviewed were valid via a DPS website search. The Written Document of Corrections is due within 10 days The following item is due within 10 days of receiving this document: A copy of the current violation free fire inspection
INSP-0041684 2024-03-19 Compliance (Annual) Complete
Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 03/19/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was given to the director at the time of the inspection. The director will email the form. The Empower Self-Evaluation was emailed to the director. The Director will email a copy of the Statement of Childcare Services and a copy of the Fire Inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Having the Art room become licensed and have a field trip form completed by parents. *Sign in and out forms have children's full name. *Multiple Purpose Room maintenance when children are present. *Using the Department Fire Drill Form for simplicity. *Ensuring that playground gates are closed to unlicensed areas. *Maintenance of garden area. *Post Lesson Plans.
