Findings: Based on the Surveyors' observations and staff statements, staff used mechanical restraint as a form of discipline. Upon arrival at the facility, the Surveyors observed a 3 year-old sitting in a high chair in the Toddler classroom. The Surveyor asked the staff member why the child was in the high chair. Staff stated the child was placed in the high chair because they were being mean to the other kids.
