Findings: In a random sample of emergency cards reviewed, specialist did not observed parent signature and date on two emergency cards, physician's information on three emergency cards and permission to pick up on one emergency card. Upon review, director showed specialist more updated completed forms for some, but not all random cards observed. Center must keep only one completed version of card for each student for clarify and safety in case of emergency. Center will have parents complete immediately and center will review all children's emergency cards to ensure they are completed in all areas as per regulations.
