| Findings: LS observed the bathrooms in rm 2 and rm 4 missing paper towels in the bathroom. The program states the children do not use the bathrooms in those rooms as they are diapered and hands are washed at the diapering sink located in the room. LS explained that since it is still a bathroom, the items are still required. Those same bathrooms had toilet paper and soap just no paper towels. LS also observed the handwashing sink in rm 4 missing paper towels. Program is to send proof of correction to OCC ASAP. | |||

| 2024-02-27 | Complaint | 13A.16.07.06A | Corrected |
| Findings: The program self reported that a teacher brought a child to the director from the Blue Horses classroom preschool and said that she saw him in the hallway alone and brought him straight to the director. The director walked back to the classroom that the child belongs in and the teacher was calling the childs name and inspecting the room from corner to corner- obviously in a panic, searching for him. Based on the details and the timeline provided, the child was in the hallway for a minute or less. LS spoke with the director that verified that this information is correct. The teacher who was in the room is not here today so LS is unable to speak with her. The program states they have a policy in place for name-to-face in the tablet that should be done at least 4x (lining up, out of the room, arriving to destination, inside of destination) to prevent these incidents. Per the program, the teacher stated she did not follow this procedure since she was only taking the children up the hallway and back. Program states the teacher will be retaking the procedure training per Lightbridge again and a supervision training through CCEI. | |||
