Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre-service trainings are completed, staff persons # 2 and # 3 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. If there are no staff person(s) available to supervise staff persons # 2 and # 3, staff persons # 2 and # 3 may not work in a child-care position at the facility.

Noncompliance Area: On 3/24/26 during a complaint inspection children # 1, # 2 and # 3, ages 5, 7 and 9 were observed getting out of a van, the children reported that they were not wearing seatbelts. Facility vans do not have booster seats. Staff # 1 was driving the vehicle.

Correction Required: A child 7 years of age or younger shall be transported in accordance with the requirements for parents and guardians as stated in 75 Pa.C.S. §4581 (relating to restraint systems).

Correction Required: A staff person shall provide to agents of the Department immediate access to the facility, the children and the files and records.

Noncompliance Area: On 3/24/26 during a complaint inspection the infant/toddler room and the preschool/school age rooms were observed to be out of ratio. Staff # 2 and # 3 were observed caring for 11 children, youngest 11 months old, oldest 2 years old. Staff # 1 was observed caring for 13 children, youngest 3 years old.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Tiered LIS: 1. Staff/child ratio will be maintained at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must create a policy in order to maintain proper ratio. This policy will ensure that proper staff: child ratio is maintained at all times. This policy must address how proper ratios will be maintained during morning drop-off and afternoon pick-up when a staff is required to answer the door for parents, how lunch breaks will be handled, and what staff should do if a parent is dropping off and they will have too many children. The legal entity must receive approval of the policy from the Regional Office. Once approved by the Regional Office, the legal entity must review policy with all staff and have staff sign off that the understand and will follow the ratio policy. The operator shall provide a correction date when policy is completed. 3. The legal entity must arrange for all staff to receive a minimum of three hours of training regarding Supervision. The training must be PQAS approved, in-person, and outside of childcare hours. The legal entity must receive DHS approval of the training content prior to scheduling the training. The operator shall provide a correction date when training is completed.

Noncompliance Area: File for Staff # 1 did not contain verification of annual fire safety training. Last one in file is dated 4/25/24.

Correction Required: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: Diaper changing pad in the Toddler room was observed to have a rip in the surface, rendering the surface unable to be sanitized.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Noncompliance Area: On 6/9/25 Staff # 1 was observed caring for 6 young toddlers in the Infant/Toddler Room for approximately 10 minutes. One child was picked up by parent during inspection allowing room to be in ratio.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Tiered LIS: 1.
