The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following was discussed, but not limited to: 1. Staff file documentation. 2. Playground maintenance. 3. Inspection reports. 4. DCS Central Registry.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 5/7/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection. The DES Contact Group size was in compliance at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified as valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *The lesson plan must include screen time in minutes. *The licensee will document when an enrolled child is absent due to a communicable disease, infestation, or an accident, and keep the documentation for 12 months. *The medical medication form must be completed in full when receiving medicine for an enrolled child. *The licensee shall ensure that there is a first aid kit on the facility premises that contains first aid supplies that meet the needs of enrolled children.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 05/14//2024 and are subject to changes pending programmatic review. The Empower Self-Evaluation was completed at the time of the inspection. The DES Contact Group Size was in compliance at the time of the inspection.
